FREE FLU VACCINATION CLINIC

Time: 9:00a.m. – 3:00p.m.
Location: Waunakee Community High School (Field
House Entrance by Old Library)
The
Waunakee Community School District is able to offer flu vaccines to everyone in
the WCSD. Thanks to Dane County Health and The Waunakee Community School
District Wellness Center we have been able to arrange for 400 flu vaccines that
we can provide, free of cost, to all residents within the WCSD.
Please
make an appointment by the method below that you qualify for.
1)
For those with Children who have Medicaid, anyone who is uninsured, American
Indian/Alaska Native, or do not have coverage for vaccines use the following

Individuals with insurance – there will be no copays! Please signup using this

We
will also need volunteers to help with guiding traffic, cleaning/sanitizing for
the medical staff doing the vaccines and handling paperwork to document the
vaccinations. If you can help from 8:30 to noon or from 11:30 to 3pm contact
